Registration for all leagues requires:

1. $50 refundable deposit (covers the first forfeit fee and is refunded if the team plays all scheduled games)
2. Completed league application (.pdf document below)
3. Roster/Waiver form (.pdf document below)

Solo Players

Solo players may contact the Deerfield Park District to be placed on a solo list. Players on this list are not guaranteed to be placed on a team. If enough solos are interested in a particular league, a team will be put together for season play. Otherwise team captains will be notified for solos to fill any vacant roster spots. Contact Jim Gariti at 847-572-2627 or jimg@dfpd.org to be placed on the solo list. Requested information is as follows:
